If you live with chronic pain, you have probably had someone imply — gently or not — that it is "all in your head." It is one of the most invalidating things a person in pain can hear, and it is wrong. So let me say this clearly before anything else: your pain is real. Real enough to reshape your days, your sleep, your mood, and your sense of the future.

And here is the part that is both true and hopeful: because pain is produced by your nervous system, your nervous system is also part of where relief can come from. The mind and the body are not two separate machines. Understanding how they work together — and how the subconscious fits in — opens a door that willpower and painkillers alone often cannot.

Your Pain Is Real — and Produced by the Brain

All pain, without exception, is produced by the brain. That is not a controversial claim or a euphemism for "imaginary" — it is basic neuroscience. When you injure yourself, sensors send danger signals toward the brain, and the brain decides, based on everything it knows, how much pain to produce to protect you. Pain is not a direct readout of tissue damage; it is the brain's protective response to perceived threat.

This is why pain and injury do not always match. People have severe injuries with little pain in an emergency, and others have significant pain with little or no detectable damage. Neither is faking. The brain is doing its job — producing an alarm — but with chronic pain, the alarm system itself can get stuck in the "on" position long after the original injury has healed.

How the Nervous System Keeps Pain Switched On

In chronic pain, the nervous system often becomes sensitized. The pain circuits, having fired for so long, become more efficient and more easily triggered — a bit like a smoke alarm re-tuned to go off at a candle. Pain researchers call this central sensitization, and it means the volume of pain can stay high even when the tissue is no longer in danger. The pain is not less real; the system generating it has simply learned the pattern too well.

And crucially, the brain does not decide how much pain to produce based on physical signals alone. It weighs context: stress, fear, past experience, beliefs about what the pain means, and emotional state. When your brain believes your body is unsafe, it turns the protective alarm up. This is where the subconscious — the seat of belief, emotion, and learned association — becomes a genuine lever on physical sensation.

The Emotional and Subconscious Layer

This is not about blame, and it is not about "thinking positive." It is that emotions and physical pain share overlapping circuitry in the brain, and chronic stress keeps the nervous system in a state of high alert that amplifies pain. Unprocessed emotion, a subconscious sense of danger, or a lifetime of bracing can all keep the body's alarm turned up.

Often there is a story the body is holding. A subconscious belief like "I am not safe," or "I have to carry everything," or the tension of a lifetime spent putting everyone else first — the same pattern I describe in how people-pleasing lives in the body — can express itself physically. Understanding that the subconscious shapes physical experience is simply the mind-body version of a larger truth: your subconscious beliefs quietly run far more than you realize.

What the Research on Hypnosis for Pain Shows

Hypnosis is one of the more strongly evidenced mind-body approaches to pain. A systematic review and meta-analysis of hypnosis for musculoskeletal and neuropathic chronic pain found that hypnosis produced meaningful reductions in pain intensity (Langlois et al., 2022). A broader 2025 systematic review and meta-analysis on the use of medical hypnosis to prevent and treat acute and chronic pain likewise concluded that hypnosis is an effective tool for pain management (NIH / PMC, 2025).

The reason hypnosis works on pain is not mysterious once you understand the model above: if pain is the brain's protective output, then changing the brain's sense of safety — and the beliefs and emotions feeding it — can change the output. Hypnosis is a direct route to that layer.

I once worked with a client living with severe, worsening chronic pain and lost mobility across their knee, back, and shoulder. There was no comfortable position — sitting, standing, and lying down all hurt — and it was slowly narrowing their entire life. They had come to dread moving their body at all.
